<p><strong>Which learnings for the future are inspired by the life, work, and words of Michael Seres?</strong>  <strong>What is Michael&#8217;s legacy ?</strong> This was the theme of our panel on the main stage,  day 1 at Giant Health, the event created and co-produced by Barry Shrier and Pr. Shafi Ahmed.  The session, proposed and moderated by Lorena McNaughtan, included Matt Eagles,  Paul Gordon,  Pr. <p>While Michael, humble and modest as he was, might not have agreed that we dedicate a panel to him, the opportunity to gather together and focus on him and his legacy, was a true treasure of a moment.  I had met Michael  in 2012, a few months after his transplant operation, in Palo Alto, a<a href="https://medicinex.stanford.edu/2020/06/05/michael-seres/">s an epatient scholar at one of the early Stanford Medicine X conferences</a>. Michael came up to me straightaway and expressed interest in speaking at Doctors 2.0 &amp; You. <strong>His unique person and story were awesome</strong>. I was convinced in the first moments and <a href="https://www.youtube.com/watch?v=s_YCJE3giRA">Michael did a dual keynote at 2013, bringing his dietitian to Doctors 2.0 &amp; You</a>. That meeting seems both yesterday and ages ago, in terms of all that has transpired since then in  the community and of course the successes of Michael&#8217;s life. While much more remains to be done, we cannot deny that progress has been made in the world of patient engagement.</p>

<div dir="auto"><strong>How do we answer the question of which learnings we take from Michael&#8217;s life?</strong></div>
<div dir="auto" style="text-align: start;">The image that comes to mind for me is Michael as <strong>an amazing, fearless surfer,</strong> who hopped on each new wave. He created an <a href="http://beingapatient.blogspot.com/">original blog, Being a Patient Isn&#8217;t Easy</a>, and got a huge number of hits. He then went on social media and through a social media connection with a physician at Mt Sinai in New York, was referred back to a surgeon at Oxford. And so Michael became, as he loved to remind us, the eleventh colon transplantee in the UK, an innovator as a patient. With that story, Michael then went on to keynote at prestigious conferences. This was followed by his becoming an entrepreneur-in-residence at Stanford Medicine X  and creating an award-winning start-up, named 11 Health. His start-up was devoted to connected objects that would make life easier for patients and professionals, and getting his startup&#8217;s product reimbursed. I remember asking him how he had worked his way through the regulatory process for the products, and he replied, in typical Michael fashion. &#8221; I googled to find out what we had to do, and then called the FDA and got answers to any questions we had. They were quite helpful.&#8221;</div>

<div dir="auto">Michael identified the opportunities and went after them, as a fearless surfer would see the waves coming.</div>

<div dir="auto">So what would Michael want us to do now? Isn&#8217;t the answer that patients continue to be the trailblazers, showing us the right way forward! Michael was not a micro-manager. I can hear him say &#8220;Just come up with some ideas and go ahead, with optimism. That&#8217;s all there is to it. And let me know how it goes.&#8221;</div>

<p><strong>On July 29th, 2020, it&#8217;s a historic moment for digital health in France, as Moovcare becomes the first medical application, a class 1 medical device, to officially achieve reimbursement status.</strong> I had the great honor of meeting the creator of this application, Dr Fabrice Denis, oncologist, in 2014, when he was getting ready to present his first results at ASCO. We had been introduced by the <a href="https://www.fhp.fr/">French Federation of Private Hospitals</a> who recommended I invite him to speak at Doctors 2.0 &amp; You, and Dr Denis had graciously accepted to do so.</p>
<p><em><a href="https://www.moovcare.com/">&#8220;Moovcare®</a> improves survival by monitoring patient symptoms on a weekly basis and alerting medical teams to the first signs of an anomaly &#8211; allowing for early intervention when the likelihood of successful treatment is highest.&#8221;</em></p>
<p><strong><em>Nine Years of Work for the Inventor and the Development of the App</em> </strong></p>
<ul>
<li><strong>Dr Denis&#8217;s work on the future application had begun in 2011 (!) with the idea that regularly collecting patient-generated data regarding symptoms could enable earlier intervention by the health system</strong>. At the time, he asked his patients to fill in paper questionnaires. Little did he know that it would take a total of nine years of research and development, submissions and meetings, for reimbursement to intervene officially.</li>
<li>In 2012, he began to work on developing an algorithm that could understand the patient&#8217;s patterns and help predict relapse.</li>
<li>In 2013, Dr Denis launched a first uncontrolled study of the future application.</li>
<li>In 2014, he reached Phase III with a randomized trial. In 2016, the trial was stopped, when an intermediary analysis showed <em>a<strong>n average benefit of 7 months of  survival</strong>,</em> 19 months with the application versus 12 months without.</li>
<li>On June 6, 2016, those results were presented at ASCO in Chicago. <strong>It was also noted that the application reduces phone calls for the physicians and leads to half as many lung scans! </strong></li>
<li><strong><em>In 2017, Moovcare was deemed a class 1 medical device in Europe.</em></strong> Simultaneously an American study on 766 patients confirmed 5 months survival rate and a device application was also presented in the US.</li>
<li>In 2018, Dr Denis published his final results.</li>
<li><strong>In April 2019, Moovcare received reimbursement approval.</strong></li>
<li>But it was not until <a href="https://www.legifrance.gouv.fr/affichTexte.do;jsessionid=3ED472B58FFF1001B7A61231B5DD1817.tplgfr38s_3?cidTexte=JORFTEXT000042165410&amp;dateTexte=&amp;oldAction=rechJO&amp;categorieLien=id&amp;idJO=JORFCONT000042164706">July 29, 2020, that this became official</a>. <a href="https://www.frenchweb.fr/chronologie-dune-application-digitale-therapeutique-en-cancerologie/382019#gsc.tab=0">(See also my history of the app here in French)</a>.</li>
</ul>
<p><strong><em>About the reimbursement</em></strong></p>
<p>The reimbursement is <a href="https://www.legifrance.gouv.fr/jo_pdf.do?id=JORFTEXT000042165740">500 Euros including tax</a>, per trimester. Only the specialist (oncologist, pulmonologist, surgeon) can determine that the patient is eligible and prescribe the application. He/she must inform the gatekeeper physician of the prescription, which is for six months but can be renewed. The patient must consent to the treatment of the data, have an internet connection and email, and use a specific login. <p><strong>This is an historic moment, not only because the application is reimbursed, but also because it is an official recognition of the value of patient-generated data in prolonging survival, and, last but not least, we also see how an app can reduce scans and physician time required.</strong></p>
<p>Again, congratulations to the inventor and the team. Now, the work is in the hands of the patients and physicians, in the real world. Good luck to all.</p>

</strong></p>
<p><em>Michael Seres touched the lives of so many people: family and friends, people he knew as a patient, people he knew professionally as a patient, those who saw him speak at an event, those who followed him online. The closer we were to the inner circle, the more pained we are that we cannot have « one more conversation » with the exceptional soul that is Michael. Today I found myself spontaneously imagining that conversation in the form of an interview. Here is what we said:</em></p>
<p>Denise: Hello Michael, I’m so glad we could connect despite the massive time zone difference. You remember our many conversations. We&#8217;d talk about how things were going and come up with ideas for the future of healthcare, digital innovation, and patients, sometimes for a post or an event, sometimes just for us. Well, I need one more conversation with you today, just one more. In fact, we all do, and I know you’re otherwise occupied, but is that ok? And then I can let everyone know how it went.</p>

<p>Michael: Of course, Denise. If I can help, I’m glad to. But first, let me ask you. Are you okay? Are you taking care of yourself? You know I worry about you. I’m so glad you’re focusing on exciting projects these days. I want you to be happy!</p>
<p>Denise: Well yes, Michael, I am. I’m really fine Michael. Don’t worry. Shouldn’t I be asking you if you aren’t working too hard on your start-up, successful as it is?</p>
<p>Michael: Not a problem. I love what I do and <a href="http://www.11Health.com">11Health</a> is doing fine, Denise. We’ve got an outstanding and dedicated team. Everyone at 11Health knows I’ve never been the founder who looks over their shoulder. So when I need to take time off, when I’m not available, they carry on perfectly. But, let’s get on to our conversation and  your questions. What did you want to talk about?</p>
<p>Denise: Michael, would it be fair to say that the single activity in which you have the greatest experience is being a patient? You started that career at age twelve, thirty-nine years ago?</p>

<p>Michael: Now that you ask, I have reached the most senior level of « patienthood » indeed! It was undoubtedly thanks to the words of the first gastroenterologist my parents ever took me to see, when I was twelve. That consultant went on to care for the Queen and became Sir Anthony Dawson. This wise gastroenterologist told me that he could help me under one condition, « that we would be a team, ie that I would need to hold up my end of the responsibilities. It’s what I’ve been doing ever since, for all my diseases, holding up my part of the bargain. I say diseases, because after Crohn’s, and the transplant, I went on to face cancer twice, and a whole set of medical situations. You know I never do things halfway!</p>
<p>Denise: Agreed, Michael. So you became quite informed about each of these conditions, through your own reading, through relationships with healthcare professionals and patient peers. You also became so experienced in hospital stays, that you were named a food representative by the NHS. You started a <a href="http://beingapatient.blogspot.com/">blog</a> to reflect upon the patient perspective, a blog which was noticed by 100 000 readers including your surgeon back in 2011-2012.</p>
<p>Michael: You have followed my story closely, Denise. Yes, I had quite a few useful remarks to give about the food served in the British hospitals. Take British cooking and add hospital and the result is not French cuisine by any means. I started that blog, when I was on my way to becoming the 11th colon transplant patient in the UK, and subsequently added in my activity on Twitter and Facebook. And following that, crowdfunding for health-related charities, through long distance bike races from England to France, and in the US. You followed a few of those races on social media, I recall.</p>

<p>Denise: Yes, Michael, and let us not forget that you became a figure on the conference circuit, which is how we met in 2012. With your transplant surgeon’s permission, you were attending Stanford MedicineX on an e-patient scholarship. I was presenting the European Wrap-up. You came up to me to enquire about speaking at <a href="https://www.doctors20.com">Doctors 2.0 &amp; You</a>. With your combination of humility, enthusiasm, and wit, I was convinced at the word “enquire”. Doctors 2.0 &amp; You was your first international speaking engagement. And you’ve made some very close friends there I can tell you.</p>
<p>Michael: Yes and I returned. How could I not love an event that offered multiple patients the opportunity to be speakers and keynotes for a global audience in the room and on Twitter? So many unforgettable people.</p>

<p>Denise: Yes, and you are so amazing that not only did you speak at multiple events around the world, despite the travel challenges of your medical condition, you even became an e-patient in residence at Stanford Medicine X. Whatever you do, you do successfully! It’s just an inescapable fact, Michael. You can’t help but put in the hard work and devotion that leads to success. And after rising to the height of communication and events, you took on the challenge of improving the patient experience through a start-up, devoted to connected objects, in a specialty niche, <a href="http://www.11health.com">11Health</a>!</p>
<p>Michael: Yes, well, someone had to do it…to put an end to dumb ostomy bags, to give control back to the patients by collecting patient-generated data, data that makes their life better and enriches our understanding of the science.</p>
<p>Denise: Yes, but not every start-up founder  creates the equivalent of 11Health and Alfred Alert, with a team, patient coaches, approved products on both sides of the ocean, multiple start-up awards. You were a key member of the global start-up panel at our last Medicine X together.</p>
<p>At this point, Michael, I have one last question: you did realize your dream, didn&#8217;t you? We all so want to know you did. From an epatient perspective, you climbed every mountain in social media and events and as an entrepreneur in digital health, you designed connected products that were approved by patient, professional, and payers &#8212; all the while being so loved and admired by all and being such a family person. Multiple lives!</p>
<p>Michael: Yes, Denise, <strong>rest assured I did realize my dream</strong>…It’s been fantastic. I could not have asked for a better, more supportive family, more amazing friends and colleagues, a more interesting existence, and a greater opportunity to help. Isn’t that why we are here? <p>The calendar is a huge success with lots of media coverage in France. At Doctors 2.0 &amp; You, we are partners with the people involved in this operation and thought that our many international friends should have the opportunity to hear about it and hopefully order one, since all donations go to the participating associations. Read on to learn more.</p>
<p>While the body is  something we are born with, many people lose confidence in theirs some time in their childhood or adolescence. They feel that some aspect of their physique is &#8220;not good enough.&#8221; We know the harmful consequences of &#8220;body-shaming.&#8221;</p>
<p><strong>Now imagine that your body is concerned by a medical condition or treatment that leads people in your environment to consider you as &#8220;different&#8221; or even inferior</strong>.</p>
<p><em>In Paris, France,  Claude Touche of <a href="https://www.evedrug.eu/#">EveDrug</a>,  a start-up founder and friend came up with a fantastic idea: a year 2020 patient association calendar, to be launched at the 30th edition of the <a href="https://festivalcommunicationsante.fr/">French Festival of Health Communication</a>. In this calendar, 12 patient associations, one per month, would &#8220;own&#8221; their condition, by baring all.</em> For the execution, he turned to Stéphanie Chevrel et Gaël de Vaumas, co-founders of a French webtv <a href="https://www.acteursdesante.fr/communique-de-presse-les-patients-se-mettent-a-nu-pour/1549/">Acteurs de Santé</a>, who chose photographer <a href="https://www.nathalie-oundjian.com/en/">Nathalie Oundjian</a>.</p>

<p>As 2019 draws to a close, let us remember an important milestone achieved this year by  <a href="https://www.conference.icpic.com">ICPIC</a>, <strong>the International Conference of Prevention and Infection Control</strong> founded by Prof. Didier Pittet, in Geneva.  <strong>For its fifth edition, <a href="https://www.doctors20.com/why-icpic2019-patientincluded-conference/">ICPIC was patient-included</a>.</strong>  This very international conference (130 countries present) opened with a memorable keynote by Vanessa Carter, which drew the full attention of the 1000+ delegate-packed auditorium. Quite an audience for Vanessa&#8217;s keynote indeed!</p>
<p>It was a fascinating first participation for me, with a very rich program prepared by the <a href="https://www.conference.icpic.com/programme/committees/">outstanding Scientific Committee</a> with the support of patients like Vanessa and others who spoke at the conference.</p>

<p><em>The Patient-Centricity and Digital Innovation Session at Frontiers Health 2019</em>, which I had the great pleasure of producing and hosting, with the support of the Frontiers Health Team and in particular Elena Pirofalo, was standing-room only. The topic is of great interest and the panel was quite diversified, with 7 speakers living in as many countries and representing patients, providers, industry, start-ups, digital consultancies.</p>
<p>A special shout-out to  Open Project Diabetes for being a truly patient-driven initiative with results. Thank you Saskia Wolf for leading the way for us at this session.</p>
<p>Each member of the session contributed a quote of his or her key messages. Thank you to each speaker for your very sincere participation, which helped maintain the attention of the audience, despite the overflow conditions. As regards the three day conference in its entirety, bravo to Roberto Ascione, Luigi Pavia, Antonietta Panella for the professionalism at Frontiers Health and the resources deployed. And it was great to meet up again with friends Eugene Borukhovich, Fredrik Debong, Paul Tunnah, of the conference committee.</p>
