The 12 digital health FrenchTech start-ups that won the challenge organized by the French Foundation of the Academy of Medicine and Doctors 2.0 & You pitched to the attendees, gathered at UNESCO (700 participants) The start-ups were selected for their concrete experience to-date and for their ability to propose a health or medical service to emerging markets. They cover a variety of topics in eHealth including: emergency telemedicine (Nomadeec) and telemedicine (Carians, DeepLink) precision radiotherapy (Therapanacea), pharmacovigilance (EveDrug), connected objects (Bioserenity, FeetMe), physician and surgical training (Invivox), professional replacements (medGo), health and wellness (Umanlife), hospital and patient communication (TelecomSanté), serious gaming (Tilak Health).

#FAMx Paris, participatory culture in service of healthcare systems

Every year since 2012, I have enjoyed participating in Doctors2.0 & You, I learned a lot at these meetings. The new “conference in a conference” format allows us to mobilize differently the #doctors20 community and to adapt itself to the development of digital in healthcare

Economic constraints are always important in healthcare systems. Reverse innovation opens the prospect of new financially sustainable progress. The program of FAMx Paris conference (31 May – 1 June at UNESCO) gives us the opportunity to discover these frugal innovations with the hope that better care and lower costs will not contradict one another. By giving a voice to healthcare professionals and researchers in emerging countries, it is also aparticipatory culture that FAMx Paris contributes to help healthcare systems progress.

Within a community, each contribution matters

The changing role of healthcare professionals is one of the three major challenges put forward by the conference. It is probably by advancing participatory culture that healthcare systems can better integrate the upheavals induced by new technologies. Jenkins (2009) describes several key elements needed to create a strong participatory culture. Low barriers to entry and strong culture are needed to create and share. In a community, members have to believe that their contributions matter and to feel some degree of social connection with one another. Taking into account the innovations produced by professionals and patients of emerging countries contributes to develop this strong culture of sharing.

In Dakar, Paris or Jaipur, internet and smartphones have seen similar developments in just a few years. The opportunities offered by these tools are still widely underused. Healthcare professionals have to take into account rapidly changing uses. Patients find it difficult to understand why so many activities of everyday life that are integrated in the digital transformation change so slowly in healthcare institutions. These evolutions are complicated to implement and experiences from elsewhere help us better understand digital transformation. The innovations that will be presented at FAMxParis will allow us to take a fresh look at issues that we cannot yet solve.

A different perspective on digital transformation

Let’s take the example of Khushi Baby. It is a connected necklace, developed by an Indian team, that facilitates the follow-up of maternity and the infant. This project was recently presented at the last Geneva Health Forum. It shows us an approach to electronic medical records that is very different from the one we know. Khushi Baby is probably not transferrable to the context of healthcare systems in Europe. But we can take a different look at connected objects and the electronic medical record after listening to Mohammad Shahnawaz tell us the story of this project.

I am impatient to discover the projects that will be presented at FAMx Paris. This conference will be the opportunity to develop this participative culture which we so need in healthcare to address the challenges of digital transformation.

Interview with Dr Armando Barriguete: how do medical training and innovation cross borders? #FAMxParis

How do medical training and innovation cross borders? According to Dr Armando Barriguete Meléndez, “it takes the generosity of real people, with a common goal, working together, and we can do great things. This has been my experience in collaborating on behalf of Mexico with France.” Dr Barriguete, president of the Franco-Mexican Medical Foundation, is an Eating Disorders and Obesity Consultant, a clinician receiving patients and collaborating with leading public institutions in Mexico and around the world.

April 16, 2018 – Dr Barriguete presents the work of the Franco-Colombian-Mexican cooperation to President Macron.

Engaged in the international activities of the Foundation of the Academy of Medicine, (FAM), Dr Armando Barriguete Melendez will attend FAMx Paris, the international forum on Reverse Innovation in Healthcare, organized by FAM in cooperation with the French National Commission to Unesco, and Doctors 2.0 & You.

Thanks to the Doctors 2.0 & You cooperation, I had the great pleasure of interviewing Dr Barriguete on his recent visit to Paris. I was impressed not only by our conversation but by the videos and articles online demonstrating that Dr Barriguete seeks to improve the quality of care provided to patients, not only through his practice, but by exchanges with the public, through academic collaborations, and with public institutions in Mexico and around the world. Dr Barriguete works with a platform of five national Mexican research institutes and two of the largest Mexican universities, Universidad Nacional Autonoma de Mexico, UNAM and the University of Guadalajara.

As part of a Franco-Colombian-Mexican delegation to France, Dr Barriguete presented the group’s finding on obesity to Emmanuel Macron in Paris on April 16^th. We see him in this photo handing over the latest report from the Franco Mexican health cooperation and nine scientific publications produced with the French National Academy of Medicine and the Academy of Surgery. Says Dr Barriguete, “This cooperation is a model for the Legion of Honor Society’s Americas Network. On May 29th, President Macron will make a speech in which he will use as a model for the international promotion of the French language, innovation in science and health in Latin America.”

Dr Barriguete is looking forward to attending FAMx Paris, whose program bringing together representatives of large emerging economies, such as China, India, Russia, Brazil, Mexico is unique. As Dr Barriguete notes, “These countries are each a continent, in terms of their size and specificities. We must congratulate the officers of the Foundation of the Academy of Medicine for their work in putting together this exceptional program at FAMx Paris.”

Indeed France, where Dr Barriguete studied both psychiatry and eating disorders, holds a special place in Dr Barriguete’s heart. He early on developed

a strong professional network with professors of medicine who shared his interest in developing international health programs. In 2006, when Dr Barriguete became an advisor to the Mexican Ministry of Health, he was in a position to bring in new programs to help Mexico improve in nutrition and in obesity prevention. Dr Barriguete’s search led him to identify France’s programs such as PNNS, Nutrinet, Nutriscore and to work on bringing them to Mexico. In 2014, the French and Mexican Presidents relaunched a strategic cooperation between the two countries, a program devoted to nutrition and obesity and adolescent health. “This was accomplished, says Dr Barriguete, with the help of the French Academy of Medicine and the Foundation of the Academy of Medicine.” Dr Barriguete also went to Colombia to develop a project with the Columbian Academy of Medicine and from there to Guatemala. He notes that all of Latin America will engage with these programs over the next decade.

There will be much more news of Franco-Mexican medical relations over the next ten years and you can be sure that Dr Barriguete will be part of what is happening. Please join him at FAMX Paris to be held at Unesco on May 31-June 1, 2018.

Interview of Prof. Mahesh Misra on reverse innovation and healthcare in India #FAMxParis

I had the great pleasure of interviewing Professor Mahesh Misra, keynote at FAMx Paris, thanks to the cooperation between the French Foundation of the Academy of Medicine and Doctors 2.0 & You. Unesco headquarters will be the site of FAMx Paris on May 31st-June 1st, 2018, organized by the Foundation of the Academy of Medicine. This international forum will bring together 400 participants from all continents. The program focuses on “reverse innovations” in healthcare, coming from key emerging economies whose healthcare institutions have partnered with the Foundation. Registration is free. Professor Misra is currently President of the Mahatma Gandhi University of Medical Sciences and Technology, Jaipur, Rajasthan, India, where he is also the Director of Surgical Disciplines and Professor of General and Minimally Invasive Surgery. Professor Misra advises the Ministry of Health on the future All India Institute of Medical Sciences*, Nagpur. Professor Misra was previously Director of All India Institute of Medical Sciences, New Delhi. He did his medical degree at Kanpur University, India and his Master in Surgery at the University of Allahabad, India.

* The All India Institutes of Medical Sciences (AIIMS) are a group of public medical schools in India.

DS Professor Misra, would you describe your main messages for FAMx Paris participants?

MM As world health becomes a global village, it is our responsibility to innovate in both diagnosis and therapy, and thereby help large populations of the world, particularly in Asia. When we speak of universal health in 21st century, we refer to the provision of health for all. So, we need to rationalize the cost of healthcare, and this requires partnerships.

We also need to develop a strategy for the prevention of various preventable diseases and focus on “low cost innovations” which can be applied to large populations in under-served geographic areas of the world. These include India, southeast Asia Bangladesh, Sri Lanka, Pakistan, Afghanistan and other Asian countries.

With these low cost or frugal innovations in healthcare, we can do the same or better work as what is currently done in Europe. And if the technologies that we develop in partnership are good for human beings in India, they are good for developed countries as well. Healthcare cost is an issue for countries like India, but also in Western Europe and the USA. How can we have accessible and affordable healthcare for everyone?

DS In Europe and the US, we are increasingly speaking about personalized medicine. What do you think of this concept?

MM Well, can we rationalize and personalize? These are two divergent issues; personalization involves costs. I think we need a reasonable healthcare, which is acceptable and affordable to the majority of the population. In India, we will personalize for a smaller segment of the population than in Europe. Who is paying? The taxpayer, the individual, the insurance company? Someone has to bear the cost.

DS You are a strong proponent of public health. Can you elaborate?

MM We study WHO projections for the diseases which are responsible for major healthcare costs in coming decades. They are: cardiovascular disease, mental health, injury and trauma in terms of DALYs (Disability Adjusted Life Years Lost). Injury is a disease of the young population. Injury in males aged 15-24 is more prevalent than cancer and heart disease. In developing countries, the number of patients who die due to injury is greater than for cancer and heart disease. We need prevention strategies to reduce mortality from injury and injury itself.

We have a paradox in India. Tuberculosis and malaria are still taking a huge toll on our lives; w e see many bacterial and viral diseases; new infections, such as dengue fever, have emerged. Plus, we have an epidemic of non-communicable diseases, such as high blood pressure, respiratory diseases (Pollution linked), diabetes. The problem of morbid obesity is on increase significantly but we have not yet won against malnutrition. We cannot ignore these major public health problems.So, we need frugal innovations in order to be able to attack all these challenges simultaneously.

DS How did you get interested in “injury”?

MM In 1988, I was awarded a commonwealth medical fellowship at the University of Manchester, UK.I chose to study trauma care services when nobody was talking about trauma as a problem. Many colleagues were laughing at my choice. They didn’t realize that it was a serious issue. Sixty thousand young lives were lost on roads in India. The only difference is that this number has grown exponentially. Look at road deaths alone. There are more than 150 000 every year (5 – 8 % increase each year). As to accidental deaths, there are more than 400, 000 in India
Injury incidence is about 30/100,000, double that of breast cancer ( incidence ranges between 14 – 29/100,000 in different cancer registries).

DS You also have an interesting experience to share regarding students. Please tell us about it.

MM Yes, I will share the experience of the i-fellowship in information technology.We have young engineers as Fellows with interest in medicine. They join and get immersed themselves in the hospital environment and shadow some of the physicians and surgeons to see the issues/problems and the potential solutions. We do this at the All India Institute’s School of International Biodesign, (Erstwhile Stanford India Biodesign). We started this fulltime two-year International fellowship around ten years ago. Our students also work with Stanford in Japan, and we would like to work with France.”

As the founder of Doctors 2.0 & You, I am delighted to announce the first cooperation for Doctors 2.0 & You as a conference within a conference, with FAMx Paris on (May 31-June 1, 2018) in Unesco headquarters. We will be co-constructing the FAMx Paris start-up challenge, running a social media ambassador team, partnering with media, and many other great activities. We hope to see many of you from the local and international Doctors 2.0 & You community. Free admission!

FAMx Paris is also a first for its founders, the Foundation of the French National Academy of Medicine (FAM). FAM x Paris is a two-day international conference bringing together participants from Brazil, Russia, India, China, Senegal, Mexico … Europe, and North America with 12 “reverse innovations,” 44 speakers, 12 start-up exhibitors chosen via an online challenge, world coffee breaks and…free entry, thanks to the sponsors.

Reverse innovations for health, the theme of FAMx Paris, reverse the direction of the flow of health innovation from low to high-resource health systems, from South to North. This theme of reverse innovation is the brainchild of FAM whose professionals have been exploring the transfer of innovations from health system to health system through bilateral symposia with each of their six country partners.

Having plunged into this theme with my knowledge of the challenges that face eHealth, I observe that innovations developed in low-resource health systems bring solutions to the people and operate with a broader mandate than in highly developed health systems. These innovations can create new roles for healthcare professionals, they can bring together technologies and techniques. What matters is that they are developed in close proximity with the endusers, the patients and the professionals.

And so, the question, we shall explore on May 31 and June 1, is how and whether we can take these innovative solutions and foster their adaptation to systems around the world, including France, the US, and others.

Take medical deserts, for example. There are 640 counties in the US that require more than an hour’s drive to reach a hospital. Can they be supported by mobile medical units or by telemedicine solutions that come from emerging health systems?
What about rocketing costs? Can “frugal innovations” from India and other FAM partner countries make our health systems more sustainable?
How can we optimize the deployment of health workers and the various health professionals?
